I. Limits of Confidentiality
Your privacy is of the utmost importance. Information shared during therapy sessions is confidential and will not be disclosed to third parties without your written consent. However, state and federal laws require exceptions to this confidentiality in the following circumstances:
- Duty to Warn and Protect: If you disclose a plan or threat to harm yourself or another person, the therapist must notify legal authorities and/or the intended victim.
- Abuse Reporting: Suspected or disclosed abuse or neglect of children, the elderly, or vulnerable adults must be reported to the appropriate state agencies.
- Prenatal Exposure: Therapists are required to report admitted prenatal exposure to controlled substances that could harm a mother or child.
- Guardianship: Parents or legal guardians of non-emancipated minors have a legal right to access the client's records.
- Insurance & Third-Party Payers: If you utilize insurance, your provider may be required to share your diagnosis, treatment plan, and progress notes to authorize payment.
II. Cancellation Policy
- Advance Notice Required: To respect your therapist's time and ensure availability for all clients, we require at least 24 hours' notice for any cancellations or rescheduling.
- Late Cancellations/No-Shows: If you miss an appointment or cancel with less than 24 hours' notice, you will be billed the full session fee.
- Fee Destination: Late-cancellation and no-show fees are released to your therapist (less payment-processing costs).
- Insurance Limitation: Please be advised that insurance companies typically do not provide coverage for missed appointments or late cancellation fees; therefore, you will be personally responsible for the full out-of-pocket cost, billed to you directly by your therapist (TMatch does not charge cards for insurance sessions).
- * Exceptions: Charges may be waived at the therapist's discretion in cases of documented illness or immediate emergency.
- Platform Status: Repeated no-shows or late cancellations may result in temporary suspension or permanent removal from the TMatch platform.
III. Guarantee of Payment
By signing this document, you acknowledge and agree to the following financial terms:
- Payment Authorization: For self-pay sessions, you are required to provide and maintain a valid credit card or payment method on your TMatch account. By providing this information, you authorize TMatch to charge your card for the full session fee at the time of booking or at a designated interval before your session begins.
- Insurance Sessions: TMatch does not process payments for insurance sessions and does not require a card on file for them. Copays, deductibles, coinsurance, and any balance your carrier does not cover remain your responsibility and are billed to you directly by your therapist.
- Fund Holding: For self-pay sessions, TMatch collects these funds on behalf of your therapist and will distribute the payment to the therapist once the session is successfully completed.
- Fee Responsibility: You remain responsible for the full cost of services rendered. Costs are determined by individual therapist rates, your insurance coverage, and session modality.
- Ancillary Charges: For self-pay sessions, you authorize TMatch to charge your stored payment method for any applicable late cancellation or no-show fees as defined in the Cancellation Policy. For insurance sessions, such fees are billed to you directly by your therapist.
